  3. Take a digital photograph, clearly showing your name and photograph of your Driver’s License OR Passport OR National Identity Card.
  4. Take a digital photograph, clearly showing your name, address, date and sender of Utility Bill or Bank Statement.
  5. Mail the documents along with your December 2019 payslip to myfutureme@aon.ie.
